Initial Comments:
Based on the findings of an offsite follow-up survey completed on September 28, 2022, Reliable Home Care LLC, was found to have corected the deficiencies cited under 28 Pa. Code, Health Facilities, Part IV, Chapter 611, Subpart H. Home Care Agencies and Home Care Registries. The deficiencies were cited as a result of and on-site unannounced State Re-licensure survey completed on May 16, 2022.
